With iSmile, the registration as a new patient is very simple and easy. We always care for our patients, that is why we focus on providing them with a hassle-free experience. We have created a form that is easily understandable and comprehensive. We believe in asking only those questions that are relevant to us and your information will remain confidential. We will not disclose your information to any third party without your consent.

For your convenience, we have New Client forms available for you to complete prior to your appointment. You are welcome to complete the forms using our online registration link prior to your visit. The link will be emailed to you prior to your visit. We want to provide you with quality service. That is why we are asking you to fill out this form prior to your appointment so we can have a better understanding of your medical and dental condition. Once you will click on the desired form you will be redirected to the form page. The form is divided into different sub-sections to ease you with form completion process. There are three different types of forms, the first one is the consent form, the second one is the iSmile Kids form and the last one is the new patient health history for adults.
